Kessler Foundation for Neglect Assessment Process
What It Measures
The KF-NAP detects spatial neglect by watching someone go about a morning rather than giving them a pen and paper assessment.
Ten categories are rated: gaze orientation, limb awareness, auditory attention, personal belongings, dressing, grooming, navigation, collisions, meals, and cleaning after meals. Each scores 0 to 3, from no symptom to severe neglect, for a total from 0 to 30. Higher means more severe. The KF-NAP is a standardized procedure for using the Catherine Bergego Scale — the scale supplies the categories, the KF-NAP supplies detailed instructions for observing and scoring them.

Licensing Notes: KF-NAP manual states that it is provided under license and strictly limited to administration by trained and certified clinicians. Manual downloads are available free from Kessler Foundation, but the training and certification requirement is the operative restriction.
Learn More: Severity bands: 0 absent, 1–10 mild, 11–20 moderate, 21–30 severe. If a category cannot be scored because of the person's condition — no wheelchair use, eating restrictions — it is excluded and the total is prorated: sum ÷ number of scored categories × 10.
