UCLA PTSD Reaction Index

The UCLA PTSD Reaction Index for DSM-5 helps clinicians understand a child or teen's history of traumatic experiences and how those experiences may be showing up as symptoms, including things like intrusive memories, avoidance, and heightened alertness. It's designed to be developmentally appropriate for school-age children and adolescents, with separate parent/caregiver and young-child versions available.

The tool maps directly onto DSM-5 PTSD criteria (intrusion, avoidance, and arousal), plus two items on fear of recurrence and trauma-related guilt, and produces category subscale scores alongside a total severity score. Occupational and mental health therapists use it in community-based, school, and outpatient mental health settings with children and adolescents who have experienced trauma, including in the context of TBI. A brief screening version is also available.

Use requires a licensing agreement, and is restricted to health services or research settings. Distributing electronic versions or translations without written permission is prohibited.
